APPLICATION PROCEDURES
1. Print and fill out the application form. A copy of the form is available here.
2. Submit the accomplished form at the parish office of the Mary Help of Christians Parish. Pay the registration fee of Php 500 (per course). Keep the receipt for verification purposes.

TUESDAY TO THURSDAY | 8:15 AM-12:00 NOON; 1:00 PM-5:00 PM
FRIDAY | 10:00 AM-12:00 NOON; 1:00 PM-5:00 PM
SATURDAY AND SUNDAY | 8:15 AM-12:00 NOON; 1:00 PM-5:00 PM
***NO TRANSACTIONS ON MONDAYS***
NOTES:
-Registration ends May 5, 2013
-Purchasing a t-shirt is NOT REQUIRED; it is only optional.
